The future First Lady admonished close friends when they joked Trump must have a small p.e.n.i.s because he has small hands, and cut in to retort: 'Don't say this - he's a real man.'
The claim is made in a new book about Melania out today called The Art of Her Deal: The Untold Story of Melania Trump by Washington Post journalist Mary Jordan.
Other revelations made in the book include the former model, 50, sharing posing tips with Trump to help him look better in photos.
And the mother-of-one found allegations of Trump's infidelity with former porn star Stormy Daniels and ex-Playboy model Karen McDougal so hard to deal with during the 2016 election that she once checked into a hotel to 'get away from her husband'.
Jordan writes when Melania was dating Trump in the late 1990s she would chat about him with her former roommate Matthew Atanian, a retired fashion photographer.
Atanian recalled he and Edit Molner, the girlfriend of model scout Paolo Zampolli, were once joking about Trump having a small penis.
Melania cut in and said: 'Don't say this - he's a real man'.
Melania told Atanian similar things about Trump's over-the-top, gilded apartment atop Trump Tower.
She gushed: 'Oh it's so beautiful. So much class. This is the way real men live'.
Trump infamously bragged about the size of his manhood during one of the Republican Presidential debates in 2016.
Senator Marco Rubio made a dig at Trump's small hands, prompting Trump to hold them up and say: 'Nobody has ever hit my hands before. Look at those hands.
'He referred to my hands - if they're small, something else must be small'.
The future President added: 'I guarantee you there's no problem'.
Jordan's book is the most in-depth portrait of Melania so far and details her improbable rise from a small town in Slovenia to become First Lady.
After marrying Trump, Melania was aware of his reputation as a ladies' man but it became more awkward after Barron's birth.
A friend recalled them both attending a party at the Playboy mansion where Melania was talking about her son and was 'extremely uncomfortable' as Trump grabbed women and held them for photos.
But it wasn't until the election that Melania understood the 'extent of his philandering' when dozens of women accused him of s.e.x.ually inappropriate behavior.
